The Clinical Trial Lifecycle

In Lesson 1.1 you met the stakeholders in a clinical trial. Now you need to understand the journey they all travel together. Every clinical trial, regardless of the disease area, country, or sponsor, follows the same structured lifecycle from the first idea all the way to the final archive.

As a clinical operations professional, you will spend your working days inside this lifecycle. Understanding it completely lets you anticipate what is coming next, prepare in advance, and avoid the reactive scrambling that causes most site-level problems.

Key idea: Clinical trials do not just happen. They move through a predictable sequence of stages. Knowing where you are in that sequence, and what comes next, is one of the most valuable skills you can develop.

Part 1

From Discovery to the Clinic: Clinical Trial Phases

Before a medicine reaches patients in a clinical trial, it has already passed through years of laboratory and animal testing called pre-clinical research. Once it clears that stage, clinical testing begins in humans. This happens across four phases, each with a different purpose and a different group of participants. Click each phase to learn more.

Phase I

Safety and Dosing

First in humans. Is it safe?

+ Expand Who: 20 to 80 healthy volunteers, sometimes patients. Goal: Find out if the drug is safe at increasing doses, how the body processes it, and what side effects appear. Setting: Usually a specialized Phase I unit, not a general hospital ward. Key metric: Maximum tolerated dose (MTD) and pharmacokinetic profile. Phase II

Efficacy Signal

Does it work? What is the right dose?

+ Expand Who: 100 to 300 patients with the target disease. Goal: Identify whether the drug has a therapeutic effect, refine the dose, and continue safety evaluation. Design: Often randomized, sometimes placebo-controlled. Key metric: Preliminary efficacy endpoints and side effect profile. Phase III

Pivotal Confirmation

Does it outperform the current standard?

+ Expand Who: Hundreds to thousands of patients across many countries and sites. Goal: Confirm efficacy vs. standard treatment or placebo, gather safety data at scale, and support regulatory approval. Design: Randomized, double-blind, multi-centre. Most of the work site teams do is in Phase III trials. Key metric: Primary efficacy endpoint (e.g. overall survival, response rate). Phase IV

Post-Marketing

What happens in the real world?

+ Expand Who: Large, diverse patient populations after the drug is approved. Goal: Detect rare side effects, study long-term safety, explore new populations, and compare with other treatments. Design: Observational or interventional. Often called post-marketing surveillance. Key metric: Long-term safety signal detection.

Most clinical operations professionals in the Arab world work on Phase II and Phase III trials, often for international sponsors or CROs. Phase III trials are the most complex, the most regulated, and the most document-intensive. That is where this course focuses.

Part 2

The Operational Lifecycle: What Happens at Your Site

The clinical phase tells you what type of trial it is. The operational lifecycle tells you what you actually do, day by day, to run it. Every site goes through the same six operational stages from the moment they are approached to the moment the last document is archived.

1 Stage One Feasibility and Site Selection

The sponsor or CRO contacts potential sites and asks them to complete a feasibility questionnaire. They want to know whether your site has the right patient population, the right infrastructure, and the right team to run the study. This is your site's job interview. Sites that respond well and honestly to the questionnaire are invited to a Site Qualification Visit (SQV).

Once the site is selected, the real preparation work begins. Regulatory submissions are made to the national authority (e.g. EDA in Egypt, SFDA in Saudi Arabia), ethics committee applications are submitted, contracts are negotiated, and the Investigator Site File begins to be built. This stage can take weeks to months depending on the country and institution. It is often the stage where timelines are lost.

The Site Initiation Visit (SIV) is conducted by the CRA. It is the formal opening of your site for enrolment. All staff attend protocol and GCP training, the Delegation of Authority Log is signed, investigational product is delivered and placed in storage, and systems access is granted. The site cannot enrol any participant until the SIV is complete and the site is formally activated.

This is the active phase of the trial. Participants are screened against the eligibility criteria, consented, randomized, treated, and followed up. The operations team manages visit scheduling, data entry, adverse event reporting, IP dispensing, and routine monitoring visits from the CRA. This stage demands the most from your team, and it is where the quality of your clinical operations is most visible.

Once the last participant completes their final visit, the trial begins to close. The CRA conducts a Close-Out Visit (COV), IP is reconciled and returned or destroyed, all outstanding data queries are resolved, and the database is locked. The ethics committee and regulatory authority are formally notified. Many sites underestimate this stage and find themselves rushing through it, which creates compliance risks.

All essential documents are archived securely for the required retention period. ICH-GCP requires a minimum of 15 years for most investigational product trials. The sponsor submits the Clinical Study Report (CSR) to the regulatory authority, and results are eventually published. Your responsibility does not end when the last participant leaves. It ends when the last document is filed and confirmed.

Where Clinical Trials Run into Trouble

Understanding the lifecycle also means understanding where things go wrong. The most common problems in clinical trial conduct are not caused by bad intentions. They are caused by gaps in understanding, poor preparation, or weak communication. Click each area to learn what to watch out for.

Start-Up Delays The most common reason a trial misses its enrolment target ►

Start-up delays occur when regulatory submissions are incomplete, ethics committee applications are submitted with missing documents, contracts take months to negotiate, or the ISF is not ready when the CRA arrives for the SIV.

What you can do:

✓Prepare a start-up checklist for every study and track each item with a deadline
✓Build a relationship with your ethics committee administrator before a study begins
✓Request regulatory timelines from the national authority early and plan backwards from them
✓Never wait for one approval before starting work on the next submission step

Poor Enrolment The most common reason a sponsor loses confidence in a site ►

Many sites overestimate how many eligible patients they can enrol. They commit to ambitious targets during feasibility and then under-deliver during the trial. This damages the site's reputation and can lead to the site being closed early.

What you can do:

✓Be honest and conservative during feasibility. Overcommitting is worse than undercommitting
✓Set up a screening log from day one and review it weekly
✓Identify screen failure patterns early and discuss with the PI and CRA
✓Communicate proactively with the sponsor if enrolment is falling behind

ISF and Documentation Gaps The most common finding during monitoring visits and inspections ►

Missing documents, unsigned forms, outdated CVs, expired GCP certificates, and incorrectly filed correspondence are among the most frequent monitoring visit findings. Each one requires a corrective action and uses time that could be spent on patient care.

What you can do:

✓Maintain an ISF completeness tracker and review it monthly, not just before monitoring visits
✓Track GCP certificate and CV expiry dates for all staff and renew proactively
✓File documents within 24 hours of receiving them, not in batches before the CRA visit
✓Treat every day as if an inspector could arrive unannounced

Late SAE Reporting The most serious compliance risk in active trials ►

Serious Adverse Events (SAEs) must be reported to the sponsor within 24 hours of the site becoming aware. Delays in reporting are one of the most serious GCP violations because they put participant safety at risk across all sites running the same trial.

What you can do:

✓Know the SAE definition in your protocol before the trial begins, not when an event happens
✓Have a written SAE reporting procedure on your wall, not buried in a manual
✓When in doubt, report. You can always follow up with more information
✓Document the date and time you became aware of the event, not just the date you reported it
