ICH-GCP: The International Standard

Clinical trials must be conducted in strict accordance with Good Clinical Practice (GCP) an internationally recognized ethical and scientific quality standard. GCP protects the rights, safety, and well-being of trial participants while ensuring that data is credible and accurate.

Compliance with GCP is not optional. Regulatory authorities from the FDA to local Arab health ministries inspect trial sites and can suspend or shut down trials and even reject drug approvals if GCP violations are found. For you as a clinical operations professional, GCP compliance is your daily responsibility.

The International Council for Harmonisation (ICH) publishes the GCP guideline known as ICH E6. The current version, E6(R2), includes an addendum focusing on risk-based monitoring and modern clinical trial approaches.

The 13 Principles of ICH GCP

1 Conduct trials in accordance with the Declaration of Helsinki ▼ Ethics

The Declaration of Helsinki is the global ethical framework for medical research on human subjects. It establishes that patient welfare always outweighs scientific or commercial interests. Every trial begins with this commitment.

2 Assess benefits and risks before and throughout the trial ▼ Safety

The benefit-to-risk assessment does not end at study start. It must be revisited continuously especially when new safety signals emerge. If risks begin to outweigh benefits, the trial must be paused or stopped.

3 Participant rights, safety, and well-being take priority ▼ Safety

Scientific and societal interests are never a valid reason to compromise participant welfare. This principle is the reason trial teams must act immediately on safety signals, even if it disrupts data collection or timelines.

4 Nonclinical and clinical information must adequately support the trial ▼ Science

Before exposing humans to an investigational product, there must be sufficient preclinical and prior clinical evidence that the trial is scientifically justified. This is reviewed by ethics committees before approval.

5 Trials must be scientifically sound with a clear, detailed protocol ▼ Science

A well-written protocol is the backbone of a trial. It defines every procedure, endpoint, eligibility criterion, and analysis plan. Without this clarity, reproducibility and regulatory review are impossible.

6 Conduct trials in compliance with the IRB/IEC-approved protocol ▼ Ethics

The ethics committee (IRB/IEC) approves a specific protocol. Any deviation from that approved protocol even a "small" one must be documented and may require re-approval. The site cannot unilaterally modify procedures.

7 Medical decisions must be the responsibility of a qualified physician ▼ Safety

Clinical coordinators and operations staff cannot make medical decisions about participants. The PI or a qualified sub-investigator must be responsible for all medical judgments including eligibility decisions and AE assessments.

8 Each person involved must be qualified to perform their role ▼ Quality

This is the foundation for the Delegation of Authority (DoA) Log. Tasks can only be delegated to people with the training, credentials, and experience to perform them. GCP training certificates, CVs, and SOPs support this principle.

9 Freely given informed consent must be obtained from every participant ▼ Ethics

Informed consent is not just a signature it is a process. Participants must understand what they are agreeing to, have time to ask questions, and consent voluntarily without pressure. Consent must be documented before any trial procedure begins.

10 All trial information must be recorded, handled, and stored accurately ▼ Data

Data integrity is critical for regulatory submissions. Source documents, CRFs, and all trial records must be legible, dated, and attributable. This is the principle behind ALCOA+ (Attributable, Legible, Contemporaneous, Original, Accurate).

11 Confidentiality of participant-identifying records must be protected ▼ Ethics

Participant data is protected by confidentiality obligations and, in many regions, data protection laws. Subject IDs replace participant names in all sponsor-facing documents. Access to identifying records must be strictly controlled.

12 Investigational products must be manufactured and stored per GMP ▼ Quality

Good Manufacturing Practice (GMP) governs the production and quality control of investigational products. At the site level, this means proper storage conditions (temperature, security), accurate dispensing records, and return/destruction documentation.

13 Quality assurance systems and procedures must be implemented ▼ Quality

Quality does not happen by accident. Sponsors and sites must have documented systems SOPs, training programs, monitoring plans, and audit processes to ensure that GCP compliance is built into every operation, not checked at the end.
