Protocol Deviations and Violations

A protocol deviation occurs when a trial activity is not conducted exactly as described in the approved protocol. Deviations are common in clinical trials what matters is how they are identified, documented, and managed.

No trial is perfect. Deviations happen. The difference between a site that inspectors trust and one they do not is whether deviations are honestly documented and corrected not whether they occurred at all.

Classification of Deviations

How to Handle a Deviation

When a deviation is discovered, the response must be immediate and documented. Here is the standard process.

Deviation Response Process 1 Identify and acknowledge the deviation

As soon as a deviation is noticed by any team member it must be acknowledged. Self-reporting is viewed favorably by inspectors and sponsors.

2 Assess participant safety first

Before anything else, determine whether the deviation poses any immediate risk to the participant. If so, take clinical action immediately and notify the PI.

3 Classify the deviation

Determine whether the deviation is minor, major, or a violation. When in doubt, escalate to the PI or sponsor for guidance on classification.

4 Document in the deviation log

Record the deviation in the site deviation log with the date it occurred, the date it was discovered, what happened, and who was involved.

5 Report to sponsor and ethics (if major or violation)

Major deviations and violations must be reported to the sponsor and, where required, to the IRB/IEC. Reporting timelines are usually specified in the protocol or sponsor SOPs.

6 Write a CAPA (Corrective and Preventive Action)

Identify the root cause and write a plan to prevent recurrence. CAPA documentation demonstrates that the site is proactively improving its processes.

Classify the Deviation

Read each scenario and classify it as a Minor Deviation, Major Deviation, or Protocol Violation.

Scenario 1 A participant's scheduled visit was due on Day 14. The participant arrived on Day 16 because they could not get time off work. The visit window in the protocol is Day 12 to Day 16. Minor Deviation Major Deviation Protocol Violation Scenario 2 A participant was enrolled in the trial despite having a medical history of kidney disease an exclusion criterion listed in the protocol. The coordinator missed it during the screening review. Minor Deviation Major Deviation Protocol Violation Scenario 3 A participant was given the study drug at a visit where their signed informed consent form could not be located. Later investigation revealed the consent was never obtained the team had assumed it was done at a previous visit.
