The Site Qualification Visit

When a sponsor or CRO reviews your feasibility questionnaire and decides your site has potential, the next step is a face-to-face evaluation: the Site Qualification Visit, or SQV. This is sometimes also called the Pre-Study Visit or Site Selection Visit.

The SQV is your site's first real opportunity to demonstrate in person what you described on paper. A CRA, or in some cases a dedicated site selection team, will visit your site to verify your questionnaire responses, meet the team, inspect the facilities, and assess whether the realities of your site match the promises you made.

The SQV is not an inspection. The CRA is not looking for reasons to reject your site. They are looking for reasons to select it. Approach the visit as a professional presentation of your site's strengths, with honest acknowledgment of any gaps and a clear plan to address them.

Part 1

What Happens During an SQV

A typical SQV runs for two to four hours and follows a structured agenda. Click each step to see what it involves and what the site team should prepare.

1 Opening Meeting with the PI and Site Team

The visit begins with a formal meeting where the CRA introduces themselves, confirms the purpose and agenda of the visit, and reviews the trial protocol at a high level with the PI. This is the PI's opportunity to demonstrate familiarity with the protocol and therapeutic area.

The CRA will assess whether the PI has actually read the protocol, can describe the main inclusion and exclusion criteria accurately, and understands the visit schedule and key assessments required.

PI should have read the full protocol before the SQV, not just the summary
Have a printed copy of the protocol on the table with key sections flagged
Prepare a brief verbal introduction of the site and team for the CRA
Be ready to discuss the patient population and enrolment estimate with supporting data

2 Facility Tour

The CRA will walk through the areas of the site that will be used for the trial. This typically includes the outpatient clinic or examination rooms where participants will be seen, the pharmacy or IP storage area, the laboratory, and the document storage area where the ISF will be kept.

The CRA is checking that the space is adequate, private, and practically capable of supporting the trial activities described in the protocol. They will note whether IP storage has temperature monitoring, whether the lab has the required equipment, and whether document storage is secure and accessible.

Ensure all areas are clean and organized before the visit
Have the pharmacist present to walk through IP storage procedures
Have calibration and maintenance records for key equipment ready to show
Confirm the document storage room is accessible but lockable

3 Document Collection

The CRA will collect or verify a standard set of documents from the site. These documents are used to populate the sponsor's TMF and to confirm that the site and team meet the eligibility requirements for conducting the trial.

Most CRAs bring a document collection checklist and will note any items that are missing or expired. Missing documents do not automatically disqualify a site, but they must be provided before site activation can proceed.

PI curriculum vitae, current and signed within the past 12 months
PI GCP training certificate, valid and not expired
PI medical licence or registration certificate
Sub-investigator CVs and GCP certificates if already identified
Laboratory accreditation certificate and normal reference ranges
Pharmacy licence and any relevant storage certifications

4 Ethics and Regulatory Discussion

The CRA will discuss the local ethics committee process in detail. They want to understand the submission requirements, meeting schedule, typical approval timeline, and any country-specific regulatory requirements that apply.

In Arab countries, this conversation often covers both the institutional ethics committee and the national regulatory authority. For example, in Egypt a trial requires both institutional IRB approval and EDA registration. The CRA needs a realistic timeline for both.

Know your institution's ethics committee meeting dates for the next three months
Have the submission checklist from your ethics committee to share with the CRA
Know the national regulatory pathway required in your country for this trial type
Be ready to give a realistic end-to-end approval timeline based on prior experience

5 Closing Meeting and Next Steps

The SQV closes with a summary meeting where the CRA shares their initial impressions, confirms any outstanding document requests, and outlines the next steps. This is not a formal decision meeting; the CRA will write an SQV Report and submit it to the sponsor, who makes the final activation decision.

The site team should use the closing meeting to ask any questions about the trial, the timeline, and what to expect next. This is also the opportunity to clarify anything discussed during the visit.

Ask for a list of any outstanding documents so you can follow up promptly

SQV Document Readiness Checklist

Before the CRA arrives, your site should have a set of documents ready to present and provide. Use the interactive checklist below to track what you have prepared.
